Please submit all forms at least 2 weeks prior to the requested date of use. After a form has been submitted, the Board of Directors will review it and notify the applicant if the request has been approved. Only groups affiliated with Immanuel Lutheran Church are permitted to request the use of the van. It is not available for personal use by outside individuals or church members.
